I fill in the gaps: helping you and your loved one navigate a complicated system (virtually), and making sure that you have access to all the right tools and resources for their highest quality of life. When needed, I will refer to someone who is able to be in-person for the Care Management your loved one may need, as well as other localized in-person services, as I no longer do the in-person Services.
