What is a Geriatric Care Manager?
The National Association of Professional Geriatric Care Managers defines a Geriatric Care Manager as "a professional who specializes in assisting older people and their families in meeting their long-term care arrangements." 
Lorraine Cummings, LCSW, C-ASWCM likes to think of a Geriatric Care Manager as "one who looks after disabled or senior persons whose families live far away and want a local advocate for their loved one's care, like an honorary Daughter."

Why Use a Geriatric Care Manager?
The most important reasons why many people choose to have a professional Geriatric Care Manager for their loved ones are to have peace of mind about the on-going care needs of their disabled or elderly family members, and to take the ever-increasing load of responsibilities off of their shoulders.  Oftentimes there is a crisis within the family that prevents the usual oversight of the elder and an outside professional becomes necessary.

What Happens When We Come Visit Your Loved One?
We begin with a comprehensive assessment where your loved one lives, conducted by a Licensed Clinical Social Worker (LCSW).  We then evaluate the older or disabled person's physical, functional and emotional abilities, and his or her living arrangements.  We identify and recommend interventions that are necessary to enhance the quality of life of your loved  one.  We also set up a plan of ongoing monitoring with regular reporting to the family or court-appointed fiduciary.  Supportive counseling for the client may also be appropriate, which is billable to Medicare.
